Introduction
These Terms and Conditions (“Agreement”) govern the use of telehealth services for weight loss, menopause, men’s hormone support, and skincare consultations provided by Elite Menopausal & Weightloss Clinic (“Provider,” “we,” “us,” or “our”). By booking an appointment, you (“Patient,” “you,” or “your”) agree to these terms.

Telehealth Services
Our telehealth appointments are conducted via video conference, phone, or other online communication platforms. By agreeing to these Terms, you consent to receiving medical consultations, advice and treatment plan remotely, which may include discussions, investigations and treatment related to weight loss, menopause, men’s hormone support, and skincare.

Eligibility
To use our services, you must be at least 18 years old, located in a region where telehealth services are permitted, and legally able to provide informed consent.

Appointment Scheduling
Appointments are subject to availability. You will receive a confirmation email or message upon booking. You are responsible for providing accurate information when scheduling and notifying us promptly of any changes. You will need to have all forms completed through the booking platform which are required for your appointment including debit/credit card details & medicare details no later than 24hrs prior to your appointment. Unless you have booked an appointment closer than 24hrs away then you will need to have all the above details entered within the hour of booking your appointment.

Fees and Payment
Payment for services must be made prior to or on completion of the appointment. Your card details must be updated into our booking system prior to attending your appointment. Accepted forms of payment include debit/credit card. If for any reason bank rejects or the payment is unable to be processed there is an additional fee of $5 upon each attempt (attempts will be conducted daily) and payment of your account needs to be finalised with 24hrs of the time of your appointment. We have provided the pricing structure on our website.

Medical Disclaimer
Telehealth consultations are intended for collect and provide information, order investigations if required and discuss options in management and a treatment plan. Our services do not replace your general health appointments with your local health practitioner. Elite Menopause & Weightloss Clinic does not guarantee any specific outcomes related to your treatment or consultation.

Confidentiality and Privacy
We comply with applicable privacy laws and regulations such as: Elite Nurse Practitioners work under the Code of Ethics for Nurses in Australia and the Code of Professional Conduct for Nurses -Nursing and Midwifery Board of Australia. (www.nursingmidwiferyboard.gov.au). They also work under the Nurses and Midwives Act 2006 2 and Nurses Code of Practice 2000. Elite Menopausal & Weightloss Clinic is dedicated to protecting your personal health information. By participating in a telehealth appointment, you consent to the collection, use, and storage of your information in accordance with our privacy policy.

Risks and Limitations
Telehealth consultations may not be appropriate for all health concerns. In certain cases, we may recommend an in-person evaluation with your regular/local health practitioner. Technical issues such as poor internet connectivity or system failures may affect the quality of the consultation.

Patient Responsibilities
You agree to:
- Provide accurate health information
- Follow the advice or instructions given by the Elite Nurse Practitioner
- Ensure a quiet, private space for the consultation
- Inform us if you have any technical difficulties before the appointment
- Notify the Provider if you experience any adverse effects or complications from the treatment

Cancellations and Rescheduling
You may cancel or reschedule an appointment with at least 24hrs notice to avoid a cancellation fee. Late cancellations or no-shows will incur a fee as detailed on our website.

Refunds
Refunds will only be provided if a consultation is cancelled by the Provider or in cases where services were not provided as agreed. Please note that there are no guaranteed results from the treatment plan developed by the Elite Nurse Practitioner, therefore no refunds will be provided in relation to treatment results. No refunds will be issued if you fail to attend the scheduled appointment.

Intellectual Property
All content provided during the consultation, including any materials, presentations, or proprietary information, remains the intellectual property of the Provider. You may not reproduce, distribute, or share any such content without permission.

Termination of Services
We reserve the right to terminate or suspend services if we believe that the patient’s conduct is inappropriate or if a breach of these terms occurs.

Limitation of Liability
We are not liable for any indirect, incidental, or consequential damages arising from the use of our telehealth services. Our liability is limited to the amount paid for the specific consultation in question.

Governing Law
This Agreement shall be governed by and construed in accordance with the laws and legislation that govern the practice of Nurses within Australia and are governed by the Australian Government and the Nursing and Midwifery Board in relation to Nursing practice. Any disputes will be subject to the jurisdiction of the courts in [your location].

By booking an appointment, you acknowledge that you have read, understood, and agree to these Terms and Conditions. You consent to the use of telehealth services for your weight loss, menopause, men’s hormone support, or skincare needs.
